My name is Laura Faulkner, and I am the Nurse at Turkey Foot Middle School. Please take a moment to read the information listed below, as it is very important.

Is your child up-to-date on Immunizations?

Please visit your student’s Infinite Campus Parent Portal Health tab to ensure immunizations are up to date. There are specific immunizations that are required by the state of Kentucky for enrollment in the public school system. This includes, but is not limited to, that all K-12 students show proof of receiving two doses of the Hepatitis A Vaccine administered 6 months apart AND students 16 years or older are required to provide proof of receiving two doses of the Meningococcal ACWY Vaccine. If the student receives the 1st Meningococcal Vaccine AFTER age 16, then only one dose is required. Students entering 6th grade will be required to provide proof of receiving the Meningococcal and Tdap vaccines. The KCSD is asking all parents/guardians to contact their child’s health care provider to ensure these guidelines have been met. If your child receives vaccines, please provide the school with a new immunization certificate. If possible, please email or fax me immunization certificates, since those brought to school don’t always make their way to me. I always recommend that you keep a copy for your records as well.

Does your child have a medication need during school hours?

If your child has a medication need during school hours there is a Medication Order Form that must be completed. This form is to be completed for both over the counter (OTC) and prescription medications. The order must be signed by both parent/guardian AND prescribing MD. If the medication is OTC, MD signature is still required. Please note that ONLY emergency/rescue medications can be carried by a student, provided proper documentation is received. This form can be located on the KCSD webpage under the Health Services tab. You will find the hyperlink on the right side of the page. These forms are also kept in the Health Office, I can supply them to parents/students as needed.

*STUDENTS CANNOT CARRY MEDICATION OF ANY KIND ON THEM AT SCHOOL WITHOUT AN ORDER THAT HAS BEEN APPROVED THROUGH THE HEALTH OFFICE*

Does your child have Diabetes?

  • Please submit MD orders

  • Please supply all necessary Diabetic supplies to be kept at school in Health Office

Does your child have a Seizure Disorder that requires an emergency medication?

  • Please submit Seizure Action Plan

  • Please complete and submit KCSD medication form

  • Please supply medication in original container with student’s name to Health Office

Does your child have a life threatening allergy that requires an emergency medication?

  • Please complete and submit KCSD medication form and please indicate if student can self-carry/administer

  • If student cannot self-carry/administer, please supply medication in original container with student’s name to Health Office

Does your child have Anxiety/Depression, ADD/ADHD, or Migraine Headaches that require medication during school hours?

  • Please complete and submit KCSD medication form

  • Please supply medication in original container with student’s name to Health Office

Does your child have any other Medical Diagnosis that affects their everyday life and will require medical awareness/accommodations during school hours? If so, please contact the Health Office.
